Physical Therapy Evaluation

Service Description

The Initial Evaluation is used to determine your functional limitations, see how your body is moving, and determine what could be contributing to your injury or complaint. This is a skilled physical therapy service that will lead to a plan of care recommendation. This could include recommended continued treatment, specific exercises, manual-based treatment, as well as general wellness and lifestyle recommendations. You will have access to the Practice Better portal where communication with myself can take place, as well as online journaling and exercise and diet tracking if desired. Payment will be due at time of treatment, but a Superbill can be provided if desired for insurance reimbursement for private insurers (Not available for Medicare). All Physical Therapy services are mobile-which means I come to you!
